The past few decades have seen an upsurge in failure analysis approaches applicable at the conceptual design stage of the product development process. These approaches mostly reason about physical failures of individual components or sub-systems. However, with the advent of highly complex systems that derive functionalities from multiple domains, more emphasis is required on understanding the mechanism of system interaction failures. This paper presents a cognitive map based root cause analysis approach applicable for system interaction failure modes identified by generating different failure scenarios. A backtracking algorithm has been developed to perform root cause analysis of the identified failures.
